Subject to change. The payment assistance component of the Viatris Originals Program is available in all provinces except Quebec, Northwest Territories, Nunavut, and Yukon. Payment assistance may vary based on your insurance plan coverage of the original brand medication.
§ Since April 15, 2021, a regulation in Quebec prevents drug manufacturers, wholesalers, and other intermediaries to provide such financial assistance in some cases. All products previously offered on the Viatris Originals card are impacted by this regulation, and as such, no patients can register for a card in Quebec.
Personal Information Consent
Personal information such as your email address, name, date of birth, address, phone number, brands of interest, and province of residence collected from you will be used by Upjohn Canada ULC (“Upjohn Canada”), a Viatris company, to send you your Viatris Originals payment assistance card, to operate the Viatris Originals Program, to send you information and updates relating to the Viatris programs such as Viatris Originals, Viatris original brand medications as well as health and wellness information that might be of interest to you. Your personal information may be shared with the members of the Viatris group of companies and trusted third parties such as service providers, agents, and suppliers in order to provide you with the products/services/information that you have requested, but will not be otherwise disclosed without your consent, except as permitted or required by law. Your personal information may also be disclosed and/or transferred to a third party in the event of a proposed or actual purchase, sale (including a liquidation, realization, foreclosure or repossession), lease, merger, amalgamation or any other type of acquisition, disposal, transfer, conveyance or financing of all or any portion of Upjohn Canada or of any of the business or assets or shares of Upjohn Canada or a division thereof.
Please note that sharing of personal information with service providers or disclosures to third parties may involve the storage or processing of personal information outside of Canada (including in the United States) where local privacy laws may require the disclosure of personal information to governmental authorities under circumstances that are different than those that apply in Canada.
By submitting your personal information, you consent to the manner of collection, use and disclosure of personal information as described above. What is a DIN?
A Drug Identification Number (DIN) is a computer-generated eight-digit number assigned by Health Canada to a drug product prior to its being marketed in Canada. It is located on the label of your medication package or on the prescription receipt.
